Insurance
Our office accepts several insurance plans. We have a contract and are providers with Delta Dental, Cigna and Anthem B/C & B/S. However, we will gladly file and accept payment from other indemnity insurance plans as long as you are allowed to visit a dentist "out-of-network." Please keep in mind, we are not affiliated with any other insurance companies than the three listed above and whatever your insurance does not pay will be your responsibility.
In addition, if you are going to have treatment performed that exceeds $250, we strongly recommend sending your insurance company a pre-treatment estimate prior to the start of treatment.
